MY HEALTH ONLINE HAS STOPPED

MY HEALTH ONLINE (MHOL) has now stopped.  You will not be able to use MHOL to book appointments or order your medication.  Please re-register on NHS App https://app.nhs.wales/login

To successfully access GP services online via the NHS Wales App, patients must:

  • Have a fully verified NHS Login, and will require a valid photo ID (passport, driving licence or national ID card) to set one up
  • An email address that isn't shared (it must be unique to that person)
  • be aged 16 or over

Any queries should be directed to the feedback channel within the app or any NHS login queries should be directed by the patient themselves to https://help.login.nhs.uk/

Guidance on registering for NHS Login can be found at https://apphelp.nhs.wales/help/nhs-login

Please make sure that you ask for a separate appointment for each person needing to see the doctor. If you are unable to keep your appointment please telephone the surgery to let us know in advance so that another patient can be booked in your place.

We now offer on-line appointment booking via the link at the top of this page. Please ask at reception for a My Health On-Line application form.

Telephone Advice

If you do not need to attend the surgery for a consultation, but would like telephone advice from the doctor, please telephone Ty-Bryn surgery and request a pre-bookable telephone appointment Monday - Friday.  These are also available to book on My Health On-Line

Home Visits

Whilst we encourage our patients to come to the surgery, where we have the proper equipment and facilities available, we do appreciate this is not always possible. In this respect, if you do need a home visit, you can help us by calling reception before 11:00.

You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable details to enable the doctor to schedule house calls

You can also be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.

Pharmacy Common Ailment Scheme

This is a service provided by your local Pharmacy who may be able to provide prescription medication without you seeing your GP. The service offers patients access to free NHS advice and treatment for common ailments that cannot be managed by self care.

Cancel an Appointment

It is important that you inform the reception staff if you are unable to attend your appointment, this will allow that appointment to be offered to another patient. If you fail to notify the Practice that you are unable to attend, you will be sent a letter informing you that you have defaulted from your appointment. Persistent defaulters may be removed from the list.

You can also complete our appointment cancellation notification request form. This can only be used if your appointment has been arranged for more than 24 hours in advance. (excluding weekends and public holidays). You can also text to cancel your appointment.

Late For Your Appointment

Please attend your appointment on time, if you are late you may not be seen. If you are not seen you will not be able to rearrange your appointment until the next working day-except in the event of an medical emergency that requires immediate attention.

Interpreter

If you require an interpreter to attend with you when you see your Doctor please notify the receptionist in advance of your appointment and they will arrange this for you.

Training Practice

Ty Bryn Surgery is a GP Training practice. GP registrars spend either 6 or 12 months at the surgery. The doctors who come to the surgery are fully qualified doctors with experience in hospital medicine and they are training to become GPs. As a patient you will be offered appointments with the GP registrar and you may also be asked if the GP registrar can video the consultation. All video recordings are strictly confidential and are used for teaching or exam purposes only.

As a practice we enjoy teaching medical students from the University Hospital of Wales. Occasionally, we may ask your permission for two of the students to be present when you are seeing the doctor. It is of enormous benefit to the students to learn from you in this way, but we will always respect your decision if you do not feel comfortable with this.
